At South Haven Health System's Rehabilitation Services, each patient receives individualized treatment from the professional staff. Our therapists listen to you. We evaluate you and your condition and plan your treatment together with you.

Our Facility & Equipment
The Rehabilitation Services department is located in the Shoreline Wellness (link to Shoreline Wellness Center) and Rehabilitation Center. This facility includes state-of-the-art equipment, a 700 square foot heated therapy pool and an indoor walking track. With this, and much more, our highly trained staff has access to tools that allow them to provide an enhanced level of care which is unique to our area.

Our Professional Staff
Once you have been evaluated by your physician and our professionals, we will determine an individualized treatment for your needs and goals. Patients are seen one-on-one with a licensed clinician. We only use licensed rehabilitation professionals; Physical Therapists, Physical Therapist Assistants, Occupation Therapists, Certified Occupational Therapy Assistants and speech Language Pathologists, who strive to meet the highly specialized needs of each patient.

For your first appointment

  • Please arrive 10 minutes early to complete any necessary paperwork.
  • Bring your physician’s prescription for therapy, along with any relevant medical information related to your injury or condition.
  • Bring your insurance cards.
  • If your condition is work-related, please bring your employer contact information.
